Our History

Caring with Grace, LLC, was founded in 2006 but really began in 2001 when my family and I became caregivers for my husband's aunt.  Soon after moving in with us, Grace was diagnosed with Alzheimer's Disease. Our family was familiar with AD because Grace's mother had also had it, and Grace had lovingly cared for her. During our journey with Grace we experienced many challenges, but also many joys.  Many things we learned the hard way, and even with my nursing background it was not easy. When I decided to start my own care management company and help other families with the challenges of caring for aging loved ones, it seemed only right to honor Aunt Grace, who was our role model for caregiving and living with grace. Hence our company name. Grace's journey with AD ended April 4th, 2008.

Meet Our Team

Angela Thomas

Angela Thomas MSN, RN, CMC

Founder and President

Angela is a master’s trained nurse with over 40 years of experience. She received her BSN and MSN from the University of Pennsylvania, School of Nursing. In 2006
Angela founded Caring with Grace in Dallas to enable her to share her passion, knowledge, and experience to assist seniors, adults with disabilities and their families
navigate the challenges of aging.


In 2016 the Aging Life Care Association® conferred upon Angela the designation, Fellow of the Leadership Academy, for her exemplary dedication to the association and the profession of Aging Life CareTM /care management. This designation signifies excellence, devotion, and dedication to serving individuals and their families in the aging community. She is an advanced professional member of the ALCA. Angela is the past president of the South Central Chapter and in 2018 she received the Outstanding Chapter Member award.
 

Angela has authored articles for The Senior Voice newspaper and speaks frequently at community events on topics related to aging. She is a member of the Case Managers Society of America, the Hospice and Palliative Care Nurses Association and the Dallas Area Gerontological Society. Angela is the past Chair of the Hoarding Task Force of Greater Dallas. She currently serves as a Director on the ALCA National Board and is co-chair of the ALCA Public Policy Committee.

Tina Lott BA, CMC

Care Manager and Director

of Business Development

Tina has over 23 years of experience working in a number of different areas including health care, public relations, non-profit, education, care management and strategic development. She earned a Bachelor of Arts in Communications from Missouri State University. Tina was executive director of the Alzheimer’s Association in South Central Kansas for 4 years and operated her own care management company for 14 years. She was one of the first educators and advocates of the person-centered care model for long term care facilities in Kansas. She also developed a “Care at Home” program and “Home Technology” program for a large hospital system. In 2014 Tina moved to Dallas with her husband. We are thrilled she has joined the Caring with Grace team as a Care Manager. Tina is an advanced professional member of the Aging Life Care Association® and the Dallas Area Gerontological Society.

Beth Hohfeler MSW, CMC

Care Manager and Social Worker

Beth earned a Master’s Degree in Social Work from the University of Arkansas at Little Rock and a Bachelor of Arts in Social Work from Baylor University. She has practiced in both the medical and non-profit arenas, with extensive expertise in oncology and chronic illness. As a care manager, Beth serves as an advocate to our clients in today’s confusing and complex medical infrastructure. She is experienced in psychosocial assessments, crisis intervention and planning for continuity of care. Beth brings first-hand professional and personal experience to the Caring with Grace team and is committed to helping our clients achieve the highest quality of life and level of independence for as long as possible. She is an advanced professional member of the Aging Life Care Association® and the Dallas Area Gerontological Society.

Beth Grzybowski MS, CCC/SLP, CMC

Care Manager and Speech-Language Pathologist

Beth earned a Master’s Degree in Speech-Language Pathology and a Bachelor’s Degree in Communication Arts and Sciences both from Michigan State University. She has practiced in both the medical and non-profit areas, with extensive expertise in neurological conditions, such as Parkinson’s disease, Stroke and Alzheimer’s. With over 30 years of experience working with the aging population, Beth is highly qualified to perform cognitive assessments, swallowing evaluations, develop an effective treatment plan and continuity of care. She is very passionate and committed to helping our clients attain and maintain the highest quality of life and level of independence in whichever setting they reside. Beth is certified in Care Management and is an advanced professional member of the Aging Life Care Association®.  She is also a member of the Dallas Area Gerontological Society.

Kristina Skipper BSN, RN, CM

Care Manager and Registered Nurse

Kristina is a registered nurse with over 30 years of experience in the clinical setting. She received her BSN from Columbia University School of Nursing in New York City. She has worked primarily in acute hospital areas including the ICU, ER, and CCU. Kristina also gained experience working for physicians, private clinics, as well as, private duty nursing care.  Most recently she has been part of a geriatrics unit in a local medical center.  Kristina's nursing skills align beautifully with our clients’ needs.  She is a professional member of the Aging Life Care Association® and a member of the Dallas Area Gerontological Society.

Amy Schulte BSN, RN, CMC

Care Manager and Registered Nurse

Amy is a BSN trained nurse with 20 years of experience. She received her nursing degree from Baylor University School of Nursing. She has worked in a variety of settings including hospital ICU, ER, hospice and Director of Nursing for skilled rehab & long term care. Her nursing case management experience comes from working for a non-profit organization where she managed the private care department with a staff of up to 60 employees. She brings both professional and personal experience to the Caring with Grace team and is committed to helping our clients achieve the optimal quality of life and maintain independence as long as possible. Amy is a professional member of the Aging Life Care Association® and a member of the Dallas Area Gerontological Society.

Charlotte Barto LMSW, CDP, CM

Care Manager and Social Worker

Charlotte earned a BA in Sociology and Anthropology from Washington and Lee University and a Master’s Degree in Social Work from the University of Texas at Austin. She has worked at Parkland Hospital as a licensed social worker where she counseled families in crisis and difficult situations. Charlotte has a wide variety of experience in leadership, serving over two decades on the boards of three different foundations. She founded a classical school for children with learning differences, with her husband and another couple. Charlotte also loves to write and volunteers her time doing copy editing and proofreading. Before joining the Caring with Grace team, Charlotte most recently worked with a local psychiatrist as a psychiatric counselor. She is an associate member of the Aging Life Care Association®, a Certified Dementia Practitioner, and a member of the Dallas Area Gerontological Society.

Jennifer Dooley BA, CDP, CM

Care Manager and Certified Dementia Practitioner

 

 

Jennifer earned a BA in nursing from the College of St. Scholastica in Duluth, Minnesota.  Upon graduation, she was commissioned into the United States Air Force and served 10 years as an Air Force Nurse.  She worked in multiple clinical settings in Texas, Germany and Idaho.  Jennifer served as the charge nurse of a large multidisciplinary outpatient clinic, managing over 6,000 patients a month.  After leaving the Air Force, she worked as the embassy nurse at US Embassy Brazil. Jennifer is a detailed person and is passionate about nutrition and wellness. She volunteers her time at the Tarrant County Medical Reserve Corps. Jennifer is a Certified Dementia Practitioner and a member of the Tarrant Area Gerontological Society.

Kristen Jackson BSN, RN, CM

Care Manager and Registered Nurse

 

 

Kristen has been an RN for nearly 20 years graduating from University of the Incarnate Word in San Antonio, TX. The first 4 years of her career were spent working in hospitals, including Methodist Hospital in San Antonio and NYU Medical Center in New York City, on post-surgical units and inpatient physical rehabilitation. Due to the strong bond she had with both her grandmothers and their experience with hospice and long term care, Kristen felt called into geriatric nursing. She has held Director of Nursing positions at Skilled Nursing Facilities for over 7 years as well as certifications in Assisted Living and Legal Nurse Consulting. She continues to consult on legal cases and in long term/home care compliance. Kristen is an active member of St. Rita Catholic Community volunteering to bring communion to the sick and homebound and educating the parish on various topics including dementia and end of life issues. Kristen is blessed with her loving husband and 4 children who keep her humble and busy. She feels truly honored to be a nurse care manager and respectfully serve the older adults in her community. Kristen is a member of the Dallas Area Gerontological Society.

Karen Cole BBA

Director of Operations and Finance

Karen is a talented business professional with over 20 years of experience specializing in information technology, project management and financial systems.  She earned a BBA in Information Systems and Quantitative Business Analysis from Baylor University.  Karen is a highly organized individual and handles a variety of daily responsibilities including accounting, billing, office management, and research.  She oversees all business infrastructure at Caring with Grace while supporting the company team and enabling them to provide quality care to our clients in Dallas and the surrounding North Texas area.
