Extending the Care Plan Beyond the Exam Room

Are you concerned about what happens when the patient leaves your office? Caring with Grace helps bridge the space between medical recommendations and the realities of the patient's daily life.

A thoughtful medical plan can become difficult to implement when a patient is also dealing with cognitive change, mobility issues, transportation barriers, family dynamics, multiple providers, or an increasingly complex living environment. That is where care management can help.

Assess

Aging Life Care Managers take a holistic, client-centered approach to assessing the home environment, existing support and resources, and family communication, and build a care plan for current and future needs.

Coordinate

We help patients understand your recommendations, coordinate next steps, identify barriers to implementation, and arrange follow-up appointments and transportation.

Monitor

We adapt the care plan to address improvements and new challenges alike. When new medications are considered or prescribed, we put a plan in place to watch for side effects, and when authorized, we communicate relevant changes and concerns to the appropriate providers.

Our role

We provide the support and services that allow for follow-through on the medical plan and preserve the patient's independence. We help families navigate the non-medical issues surrounding the patient's health so the medical team can stay focused on clinical care.

When there is concern that orders are not being followed up on or medications are not being taken as prescribed, or when health and safety concerns exist outside the office.

Home Care Agencies

Two Different Roles. One Client.

Caring with Grace is not a home care agency. We provide professional care management that complements the hands-on support your team provides in the home. With a shared focus on the client, we help create greater continuity of care, more coordinated support, and clearer communication with families and other professionals.

Assess

We help the family understand the need, define the goals of in-home care, and find the right fit in scheduling and staffing.

Coordinate

We work to ensure that the client and all involved in their care, such as family members and healthcare providers, are working together and schedules are running smoothly.

Monitor

We communicate goals, progress, and changing needs to the other professionals and family members involved in care decisions.

Our role

Your team provides hands-on care in the home: companionship, regularly scheduled support, safety, and assistance. We provide care planning, medical coordination, care transitions, and family communication. We can also serve as a local professional contact for distant families, help navigate challenging family dynamics, and ease the adjustment to care in the home.

When your client is struggling to adjust to help in the home, or needs support with family dynamics, medication management, or getting to medical appointments.

Senior Living Professionals

Supporting Your Residents as a Trusted Partner

Senior living communities provide an essential foundation of care, safety, and connection. Caring with Grace adds a layer of individualized advocacy, coordination, and professional care management when a resident's situation requires more. We bridge the gaps among the resident, the family, the community, and the other professionals involved, so that all of them can work together as one.

Assess

Assessing the appropriate level of care is key to a successful transition. Complex medical needs, social engagement, and proximity to the medical team or family are all important considerations when choosing a community.

Coordinate

We coordinate needed care within the community and beyond, including appointments with physicians and specialists as healthcare needs change.

Monitor

We monitor for gaps and changes in client needs, staffing, and other circumstances, and provide communication support among the resident, family, and community when needed.

Our role

The community provides the resident's daily care, support, services, safety, and connection. We provide individualized assessment, advocacy, medical coordination, and family communication, keeping distant or overwhelmed families updated and offering a consistent local presence for residents without nearby support.

When a resident or family would benefit from a trusted professional who can ease the stress of transitioning to a care community, serve as a local contact for distant family members, and build an ongoing, collaborative relationship with your team.
