You're paired with a care manager and will complete the same thorough assessment every Caring with Grace client receives. If your care manager confirms your situation is stable, you move into a lighter-touch, lower-cost relationship: a pre-determined schedule of visits, and a direct line to your care manager whenever you need it.

An in-depth first visit covering your health, home, goals, and support network, used to confirm this level of service is right for you.
Once your care manager confirms your situation is stable, you move into a lighter-touch relationship: less frequent, planned visits at a lower cost, with a team that already knows you.
We will revisit your goals, catch small changes early, and keep your plan current.
Something come up between visits? Reach your care manager directly by phone or email. No waiting for the next scheduled check-in.
If your needs grow, your care manager already knows your history and can step up support immediately, including moving you into our standard care management relationship.

There is no membership fee. All time with your care manager is billed at our current hourly rate, so you are only billed for the time you need.
